*For those paying with cash, money order or cashiers check please do the following:

1. To Register By Phone: Call (678)799-0159. Print out and fill out this registration form and have available your credit card information.

2. To Register By Mail: Print out and Fill out this registration form, and mail it with your cash, cashiers check or money order payable to: "Georgia Sports Leagues." Send this to: 1486 Vernon North Dr. Dunwoody, GA 30338. (Payment must be received prior to start of the season)
